Integrity Dental Services is here to help in any way you need. You can send us digital cases anytime or schedule a pickup for cases with impressions, models, and additional case-related items. Please be sure to include a relevant Integrity Dental Services Rx Form, if you do not have one you can download one here.
Sending a Digital Case
Digital files can be sent through our website here. Upload completed prescription and files.
Sending a Physical Case
If you are on a pick-up route or have your cases picked up, please fill out this pick-up request form. If you are sending your first physical case, please call us or submit a new customer form here.
Shipping
Integrity Dental Services now offers a digital way for customers to print out shipping labels for sending cases and requesting pickups from UPS. If you have any questions feel free to call us or email us.

Accepted Digital Scanning Platforms and Systems.
To begin sending files from your Medit i500 or i700 you will first need to type in the lab’s name in the search bar, click on the “Request Partnership” button. After that you will need to wait for your new partner to accept your request. Once the lab accepts your request, you will both be listed as partners on each of your accounts, and you can begin to send your orders to the lab. Please contact us for any assistance you may need for establishing this connection.

To send a case through Sirona Connect, follow the standard Sirona scan protocol and steps. Select preferences and send scans prepared with surrounding area relevant to the restoration.
Primescan and CEREC is a product of Dentsply Sirona
Adding Integrity Dental Services as a preferred Lab.
- Go to https://portal.3shapecommunicate.com and login
- From the menu, choose “Integrations”
- Locate Integrity Dental Services and choose “Enable Integration”
